Kindergarten Immunization, Vaccination Information

If your child will be entering kindergarten for the 2019-2020 school year, please review the required vaccinations below. Immunizations must be current prior to enrolling in school. If your child is missing any of these requirements, consult with their health care provider. Your child will not be enrolled in a class if they are not in compliance with the Texas Immunizations Provisions.

Kindergarten Vaccination Requirements:

5 doses of DTaP/DTP (4 doses if the 4th dose was given on or after the 4th birthday)

4 doses of Polio (3 doses if the 3rd dose was given on or after the 4th birthday)

2 doses of MMR (first dose received on or after first birthday, and second dose on or after 4th birthday)

3 doses of Hepatitis B

2 doses of Varicella (first dose received on or after first birthday)

2 doses of Hepatitis A (second dose must be given 6 months apart)
