- Student Health Information (English/Spanish) - Fill out if your student has are any new allergies or health conditions.
- Ethnicity and Race Questionnaire (English/Spanish) - Fill out if you would like to change this information regarding your student.
- Family Educational Rights and Privacy Act form (FERPA) (English/Spanish)- Fill out if you would like to change your student's directory information.
- Transportation forms (English/Spanish)- Fill out if your student plans on riding the bus.
- Free and Reduced Lunch Forms- Fill out to apply for free and reduced lunch
- Drug Testing Consent Form (English/Spanish)- Fill out if your student is in the 7th grade or above and will participate in extra-curricular activities, or will be driving themselves to school.
